<p class="MsoNormal">We are seeking a dedicated andcompassionate Case Manager to provide essential support to individuals andgroups in need. This role involves offering direct patient education,counseling assistance, and connections to community resources and services. TheCase Manager will play a critical role in supporting patients within abehavioral health setting, ensuring they receive the care and resourcesnecessary to improve their well-being.<o:p></o:p></p><p class="MsoNormal">Key Responsibilities:<br>The Case Manager will provide direct patient education and guidance related tobehavioral health care and resources. They will connect individuals and groupsto counseling services and other community support systems while assistingpatients in navigating available resources to address their behavioral healthneeds. Additionally, the Case Manager will coordinate and document patient careplans and follow up to ensure continuity of care. Collaboration withmultidisciplinary teams to develop and implement individualized care plans isalso an essential aspect of this role.<o:p></o:p></p><p class="MsoNormal" style="margin-bottom:0in"><b>RequiredQualifications:<o:p></o:p></b></p><p class="MsoNormal" style="margin-bottom:0in"><b><o:p> </o:p></b></p><ul style="margin-top:0in" type="disc"> <li class="MsoNormal" style="margin-bottom:0in;mso-list:l1 level1 lfo1; tab-stops:list .5in">Bachelor’s degree in a behavioral health field.<o:p></o:p></li> <li class="MsoNormal" style="margin-bottom:0in;mso-list:l1 level1 lfo1; tab-stops:list .5in">Minimum of two years of experience providing direct case management services to patients in a behavioral health setting.<o:p></o:p></li> <li class="MsoNormal" style="margin-bottom:0in;mso-list:l1 level1 lfo1; tab-stops:list .5in">Valid Fingerprint Clearance Card.<o:p></o:p></li> <li class="MsoNormal" style="margin-bottom:0in;mso-list:l1 level1 lfo1; tab-stops:list .5in">Driver’s License and Proof of Insurance (if requesting mileage reimbursement).<o:p></o:p></li></ul><ul style="margin-top:0in" type="disc"> <li class="MsoNormal" style="margin-bottom:0in;mso-list:l0 level1 lfo2; tab-stops:list .5in">Master’s degree in a behavioral health field preferred. <o:p></o:p></li> <li class="MsoNormal" style="margin-bottom:0in;mso-list:l0 level1 lfo2; tab-stops:list .5in">At least one year of direct case management experience related to mental health diagnoses and other related disorders preferred.<o:p></o:p></li> <li class="MsoNormal" style="margin-bottom:0in;mso-list:l0 level1 lfo2; tab-stops:list .5in">One year of experience providing patient care in a behavioral health setting preferred. <o:p></o:p></li></ul><p class="MsoNormal" style="margin-bottom:0in">Nestled in the scenic high desert of southern Arizona, Sierra Vista offers aunique blend of small-town charm and modern amenities.
