tab-stops:list .5in">Coordinate referrals and connect patients to appropriate medical, behavioral health, and community-based services.<o:p></o:p></li> <li class="MsoNormal" style="margin-bottom:0in;mso-list:l1 level1 lfo3; tab-stops:list .5in">Engage patients using trauma-informed, strengths-based, and motivational interviewing approaches to support recovery and self-management.<o:p></o:p></li> <li class="MsoNormal" style="margin-bottom:0in;mso-list:l1 level1 lfo3; tab-stops:list .5in">Monitor patient participation, follow up on missed appointments, and address barriers to treatment.<o:p></o:p></li> <li class="MsoNormal" style="margin-bottom:0in;mso-list:l1 level1 lfo3; tab-stops:list .5in">Collaborate closely with medical providers, psychiatric providers, counselors, and external partners to support integrated care.<o:p></o:p></li> <li class="MsoNormal" style="margin-bottom:0in;mso-list:l1 level1 lfo3; tab-stops:list .5in">Assist, under clinical supervision, with intakes, assessments, diagnoses, and development and maintenance of treatment and service plans.<o:p></o:p></li> <li class="MsoNormal" style="margin-bottom:0in;mso-list:l1 level1 lfo3; tab-stops:list .5in">Monitor patient symptoms and treatment responses and communicate changes to the clinical care team.<o:p></o:p></li> <li class="MsoNormal" style="margin-bottom:0in;mso-list:l1 level1 lfo3; tab-stops:list .5in">Support transitions of care, including coordination with residential treatment facilities and discharge planning.<o:p></o:p></li> <li class="MsoNormal" style="margin-bottom:0in;mso-list:l1 level1 lfo3; tab-stops:list 1.0in">Associate’s degree in a behavioral health field with 2 years of behavioral health experience, OR<o:p></o:p></li> <li class="MsoNormal" style="margin-bottom:0in;mso-list:l0 level2 lfo1;
